- Rated hollow-wall anchors as a last resort. Purpose-made heavy-duty grab bar anchors exist for situations with no stud access, and quality ones carry a stated load rating. They are better than nothing and worse than wood. If the wall is tile over cement board, use anchors rated for that assembly, and follow the anchor manufacturer's rating rather than assuming.
Tile adds one more step: drill through tile with the correct bit before driving fasteners, and seal penetrations in wet areas so water cannot reach the wall cavity. If any of this is outside your comfort zone, this is a job worth paying a handyman or contractor to do right. A grab bar that pulls out of the wall during a fall is worse than no bar, because the user has already committed their weight to it.
